If the team is performing better than their average performance then you are doing a good job as a coach, leader, mentor, teacher and so on.
If the team is underperforming, then it is time for the top person to take a deep look at himself/herself and reorient oneself to correct.
The only gauge is the results and the direction in which the results are going over a period of time. When results are not good, look at the process. Don't be so obsessed with the process especially if the results are not showing.
